Skilled nursing care, physical therapy, occupational therapy, speech therapy, medical social worker, nutritional counseling, home health aides, Child Health Conferences, Maternal Child Health Services, Certified Hospice. Referral Process: Anyone residing in Somerset, Middlesex or Warren Counties who is under the care of a physician and requires home health or hospice care may be referred for services by themselves, a family member, friend or physician, hospital, sub-acute facility or a community agency. Fee varies according to service and is usually covered by health insurance, managed care provider, Medicare, and Medicaid. Sliding scale fee available.
